In Bangladesh, this issue is becoming more important than ever. The healthcare sector is growing rapidly, and hospitals must manage medicines, surgical items, diagnostic reagents, medical equipment, oxygen, implants, consumables, and emergency supplies with greater accuracy and transparency.

According to The Business Standard, Bangladesh has 654 government hospitals, more than 5,000 private hospitals and clinics, and 10,675 privately registered diagnostic canters, supported by more than 500 regular suppliers of essential medical supplies. This scale shows why transparency in healthcare procurement and supply chain is not optional anymore—it is essential.

This is where blockchain technology can play a powerful role.

Why Transparency Matters in Hospital Supply Chain

In a hospital, lack of transparency can directly affect patient care. If a lifesaving medicine is unavailable, if a surgical item is delayed, or if expired products remain unnoticed in inventory, the impact is serious.

Hospital supply chain teams deal with many daily challenges:

  • Stock-outs of critical medicines
  • Excess inventory and dead stock
  • Delayed procurement approvals
  • Vendor performance issues
  • Manual documentation
  • Price variation
  • Counterfeit or low-quality products
  • Poor visibility between stores, pharmacy, procurement, finance, and user departments

In Bangladesh, public hospital procurement is monitored through the Ministry of Health and Family Welfare, while private hospitals usually manage procurement independently. This creates different levels of control, documentation, and visibility across the healthcare system.

From my professional point of view, transparency is not only about seeing data. It is about trusting the data.

What Blockchain Can Do for Hospital Supply Chain

Blockchain is a digital ledger where transactions are recorded securely, time-stamped, and difficult to alter. In hospital supply chain, it can create a transparent record of every step—from requisition to purchase order, supplier delivery, quality check, store receipt, department issue, and final consumption.

Instead of keeping records in separate systems or paper files, blockchain can help hospitals create a shared and trusted source of information.

For example, when a hospital purchases surgical gloves, the blockchain record could show:

  • Approved vendor
  • Purchase order details
  • Batch number
  • Manufacturing and expiry date
  • Delivery date
  • Quality inspection status
  • Store receipt
  • Department-wise usage
  • Payment status

This kind of visibility can reduce disputes, improve accountability, and support better decision-making.

Bangladesh Hospital Example: Medicine and Consumables Tracking

Imagine a private hospital in Dhaka purchasing emergency medicines, syringes, IV cannulas, reagents, and surgical consumables from multiple vendors.

In a traditional system, procurement may have one record, store may have another, pharmacy may maintain separate stock data, and finance may wait for manual invoice verification. If there is a mismatch, staff must spend hours checking files, emails, delivery challans, and invoices.

With blockchain, each transaction can be recorded in one trusted chain. Procurement, store, pharmacy, finance, and management can view the verified status based on permission level.

This can help answer important questions quickly:

  • Was the item delivered on time?
  • Was the correct batch received?
  • Was quality inspection completed?
  • Which department consumed the item?
  • Is the supplier payment justified?
  • Is there any unusual price or usage pattern?

For Bangladeshi hospitals, especially those handling high patient volumes, this can bring major improvements in operational control.

Blockchain and Public Healthcare Procurement in Bangladesh

Bangladesh already has digital initiatives in healthcare supply chain. The Ministry of Health and Family Welfare’s Supply Chain Management Portal is described as a web-based portal accessible to MOHFW, procuring entities, line directors, hospitals, drug administration, and stakeholders. It includes features such as product catalog, procurement planning, procurement package tracking, and linkage with the drug registration database.

This shows that Bangladesh is already moving toward more structured digital supply chain visibility.

Blockchain can be a future layer on top of such systems to make records more tamper-resistant and auditable. For example, procurement plans, tender approvals, supplier selection, delivery confirmations, and payment milestones could be recorded in a way that improves trust among hospitals, vendors, government bodies, and auditors.

USAID’s Global Health Supply Chain Program also reported that technical recommendations helped Bangladesh’s DGHS reduce procurement lead-time by aligning approval authority with prescribed procedures. This highlights how process improvement and governance can directly affect healthcare supply chain performance.

Blockchain cannot solve every problem alone, but when combined with good governance, it can strengthen transparency further.

Reducing Counterfeit and Substandard Products

One major concern in healthcare supply chain is product authenticity. Hospitals must be confident that medicines, implants, reagents, and devices are genuine and safe.

Blockchain can help track products from manufacturer to distributor to hospital store. Each movement can be recorded with batch details, expiry date, and verification status.

For example, if a hospital in Chattogram receives cardiac stents or orthopedic implants, blockchain-based tracking could help verify:

  • Manufacturer identity
  • Importer or distributor details
  • Batch and serial number
  • Regulatory approval information
  • Delivery history
  • Final usage record

This level of traceability can protect both hospitals and patients.

Better Inventory Visibility for Hospitals

Hospital inventory is complicated because demand is unpredictable. Emergency departments, ICUs, operating theatres, dialysis units, laboratories, and pharmacies all need different types of items.

A blockchain-enabled inventory system can provide stronger visibility into stock movement. It can show how items move from central store to sub-store, then to user departments.

In Bangladesh, where hospitals often face pressure from high patient load, this visibility can help reduce both stock-outs and overstocking.

The MOHFW Supply Chain Management Portal includes logistics tools such as eLMIS systems for monitoring commodities, including COVID-19 commodities and TB commodities. This reflects the practical importance of digital logistics monitoring in Bangladesh’s health system.

Blockchain could support similar visibility with stronger record integrity.

Benefits of Blockchain for Bangladeshi Hospitals

For hospitals in Bangladesh, blockchain can support supply chain transparency in several ways:

1. Stronger accountability : 

Every transaction can be recorded with user identity, time, and approval status.

2. Faster audit process

Auditors can review verified records instead of depending only on paper documents.

3. Improved vendor management

Hospitals can track supplier delivery performance, quality issues, and payment history.

4. Reduced fraud risk

Tamper-resistant records make it harder to manipulate procurement or inventory data.

5. Better patient safety

Traceability helps identify expired, recalled, or questionable products faster.

6. Improved financial control

Finance teams can verify invoices against purchase orders, goods receipt, and quality approval.

Challenges of Blockchain Adoption

Of course, blockchain implementation will not be easy. Hospitals must consider:

  • Cost of technology
  • Staff training
  • Integration with existing ERP or inventory systems
  • Data privacy
  • Vendor participation
  • Government and regulatory alignment
  • Change management

Most importantly, blockchain cannot fix poor data entry. If wrong information is entered, the system may still preserve wrong data. So hospitals must improve data discipline first.

In my view, blockchain should not be introduced as a fashionable technology. It should be introduced where transparency gaps are real and measurable.

Practical Starting Point for Bangladesh

Bangladeshi hospitals do not need to begin with a full blockchain transformation. They can start with pilot projects.

Good pilot areas may include:

  • High-value implants
  • Life-saving medicines
  • ICU consumables
  • Laboratory reagents
  • Emergency procurement
  • Vendor delivery tracking
  • Expiry and batch tracking

A private hospital group could start by tracking selected critical items across procurement, store, pharmacy, and finance. A public hospital project could begin with essential medicines or medical equipment procurement.

The goal should be simple: increase visibility, reduce manual disputes, and improve trust in supply chain records.

Final Thoughts

Hospital supply chain management is directly connected to patient care, financial performance, and institutional reputation. In Bangladesh, where healthcare demand is rising and hospital operations are becoming more complex, transparency is a strategic necessity.

Blockchain can help hospitals move from manual trust to digital verification. It can improve traceability, accountability, audit readiness, vendor control, and inventory visibility.

However, technology alone is not enough. Success requires skilled supply chain professionals, strong governance, accurate data, trained users, and leadership commitment.

Based on practical experience in hospital supply chain, finance, procurement, and project management, my belief is simple: the future of healthcare supply chain in Bangladesh must be more transparent, integrated, and data-driven.

Blockchain can be one of the strongest tools to make that future possible.
